If you send the contact form, we receive your name, email, phone number, how you want us to reach you, when, and your message. Please don't include medical details in a message. Your health history goes on our private patient portal, which has its own privacy terms.

What we do with it

We use what you send to reply to you and book your visit. We keep your records private and share them only as the law allows. We don't sell your information.

What this site doesn't use

This website uses no analytics, advertising pixel or session recording, and it has no embedded scheduler. Our booking page is a link to the practice's own scheduling system, which has its own terms.

Your choices

You can ask us to correct or delete a message you sent, and to stop contacting you. Call us or send a request through the contact form.
